Transitioning Best Practice to Clinical Integration

The CHCAMS will host their Spring One-Day Clinical Conference for the community health centers’ multi-disciplinary care teams providing care to patients of all ages. In so doing they are compelled to enhance primary care capacity to improve patient outcomes and integration of care across the primary care continuum. Health Resources and Services Administration (HRSA) has clinical priorities that include Ending the HIV Epidemic, Diabetes Prevention and Control and Care Integration.  CHCAMS has a mission to provide the TA/training needed to assist in addressing these priorities. Conference sessions will address special population programs, initiatives, and evidenced-based guidelines to improve primary care capacity and patient outcomes.
