FQHCs are committed to providing exemplary patient care with the desired result of favorable patient outcomes health centers are called to provide evidence-based diabetes management and utilize multidisciplinary care teams to improve patient outcomes. CHCAMS has a mission to provide the TA/training needed to assist in this process. This session will address diabetes clinical management guidelines, workforce, social determinants of health barriers to diabetes care. The Commission on Dietetic Registration has approved 5 CPE hours .

The Mississippi Board of Pharmacy has approved 3 hours of live pharmacists CE.
This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the Community Health Center Association of Mississippi.  The Community Health Center Association of Mississippi is accredited by the Mississippi State Medical Association to provide continuing medical education for physicians.
