Special Populations

The Health Resources and Services Administration (HRSA) guidance for PCA cooperative agreements requires Primary Care Associations (PCAs) to develop strategies for addressing the unique health needs and barriers to care for Special Populations in the state/region including identifying a Special Populations Point of Contact.

The targeted areas of special populations required by HRSA include: Seasonal and migrant farm workers, homeless populations, and individuals in public housing. NCQA PCMH requires that CHCs address needs of vulnerable populations as part of a quality improvement plan. CHCAMS provides special populations assistance by establishing and maintaining relationships with state and national organizations representing these patients.
